How often should you use a sauna? The number the research keeps landing on is four to seven times a week, and the effect attached to it is large enough that you should be a little suspicious of it.
In a group of Finnish men followed for two decades, those who used a sauna four to seven times a week died at roughly 40 percent lower rates than those who went once. That beats most prescription drugs. It also comes from watching people who chose their own habits rather than assigning anyone to anything, and that distinction turns out to be the whole story.
What the Sauna Evidence Actually Is
Nearly everything you have read about saunas traces back to one study: the Kuopio Ischaemic Heart Disease Risk Factor Study, a population sample from eastern Finland examined between 1984 and 1989. Researchers asked men how often they used a sauna and how long they stayed in, then followed them for decades and counted what happened.
That design is a prospective cohort, and it has a permanent ceiling. Nobody was assigned to a sauna. Men who went four times a week chose to, and they differ from once-a-week men in ways a questionnaire cannot capture: likelier to own a house with a sauna in it, to have free evenings, to be well enough to sit in 80 degree heat without trouble.
To their credit the researchers adjusted for age, body mass index, systolic blood pressure, LDL cholesterol, smoking, alcohol, previous heart attack, type 2 diabetes, cardiorespiratory fitness, resting heart rate, physical activity and socioeconomic status. That covers the two variables people usually complain are missing. Adjustment shrinks confounding. It does not delete it. And throughout, the sauna is the traditional Finnish dry one, run hot at low humidity: not a steam room, not an infrared cabin.
The Science: How Often Should You Use a Sauna?
The dose-response runs from one session to four
- The Evidence: A prospective cohort of 2,315 Finnish men aged 42 to 60 was followed for a median of 20.7 years, during which 929 died. Against one session a week, the adjusted hazard ratio for death from any cause was 0.76 (95% CI, 0.66 to 0.88) at two to three sessions and 0.60 (95% CI, 0.46 to 0.80) at four to seven, P for trend below .001. Sudden cardiac death showed the steepest gradient, 0.37 (95% CI, 0.18 to 0.75) in the most frequent group.

Two details the headlines dropped. Only 201 of those 2,315 men were in the four to seven group, so the estimate rests on 62 deaths and the interval around it is wide. And the authors ran the obvious sanity check: excluding men who died in the first five years, and capping follow-up at ten years, left the associations intact. That handles the natural objection that sick men stop going.
Session length, and the place the pattern breaks
The same paper recorded how long men stayed in, and here the story stops behaving.

Sit with that. If longer sessions bought extra years, duration should move all-cause mortality the way frequency did, and it does not move it by a hair. Either heat acts on cardiac death specifically, or the frequency finding is partly measuring what kind of man goes four times a week. The paper cannot separate them.
One cohort wearing four different hats
Here is the part that gets lost. The sauna literature looks like a stack of independent confirmations and mostly is not.
The finding that four to seven weekly sessions came with a 66 percent lower risk of dementia and a 65 percent lower risk of Alzheimer’s disease is from the same 2,315 men over the same 20.7 years, published two years later (Age and Ageing, 2017; hazard ratios 0.34 and 0.35). The stroke result, 0.38 (95% CI, 0.18 to 0.81) for the most frequent users, comes from 1,628 men and women in the same Kuopio study at a later examination that added women. So does its cardiovascular mortality analysis, and so does the work linking sauna frequency to C-reactive protein, the same low-grade inflammation that compounds like quercetin are aimed at.
Four headlines, one population, one city. If some unmeasured difference separates frequent from infrequent sauna users in Kuopio, it bends every one of those results the same way. They are not four votes. They are one vote counted four times.
What happened when researchers randomized it
- The Evidence: Forty-one adults with stable coronary artery disease, mean age 62, were randomized to eight weeks of sauna four times a week at 79 degrees for 20 to 30 minutes, or to a control group. This is the closest anyone has come to testing the cohort’s own prescription. Flow-mediated dilation did not differ between groups (P = .909), nor did pulse wave velocity (P = .816), systolic blood pressure (P = .951) or diastolic blood pressure (P = .292). Participants did acclimate to the heat, so the dose landed.

Not every trial is null. A smaller one assigned 47 sedentary adults with a cardiovascular risk factor to eight weeks of exercise plus a 15 minute post-exercise sauna, exercise alone, or nothing. Adding the sauna lowered systolic blood pressure by 8.0 mmHg against exercise alone (95% CI, 1.4 to 14.6; P = .02) and raised peak oxygen uptake by 2.7 mL/kg/min (P = .03). With 15 people in the sauna arm and no blinding, that is a signal, not a finding.

Pooled, the randomized evidence is modest. A 2025 review of 20 passive-heating trials found systolic blood pressure fell 4.11 mmHg (95% CI, 0.86 to 7.36) with whole-body heating, while diastolic pressure, flow-mediated dilation, pulse wave velocity, resting heart rate, fasting glucose, HbA1c and every lipid measure came back non-significant. Most trials carried a moderate to high risk of bias.

Heat reliably drops your blood pressure a few points. No trial has shown it extends life, and none will, because that trial needs thousands of people and thirty years.
A Practical Guide: How Often, How Long, How Hot
Four sessions a week is the target worth aiming at, because that is where the association sits. Two or three still tracks with a smaller effect, and if that is what fits your week, it is not wasted.
Fifteen to twenty minutes is the session to build toward, worked up to rather than started at. The cardiac signal appears above 19 minutes, with the caveat that duration did nothing for overall mortality. Keep it hot and dry if you are copying the evidence: infrared cabins run far cooler and have no comparable long-term data, which makes them unstudied rather than useless.
Two touches worth stealing. Put the sauna after your workout, the sequence the one positive trial used, and rehydrate properly afterwards. A single 30 minute session at 73 degrees dropped systolic pressure from 137 to 130 mmHg in 102 adults with a cardiovascular risk factor and softened arterial stiffness on the same carotid-femoral measure that vitamin K2 moved over three years. A real response, just not evidence that it accumulates.
Safety and Advice
Dying in a sauna is rare and mostly preventable. A Finnish forensic review of every sauna death from 1990 to 2002 put the annual rate below two per 100,000 people and found alcohol involved in half of all cases. That gives you the most useful rule here: do not drink and sauna. It is the dominant risk factor in the fatality data, not a footnote.

Unstable angina and a recent heart attack are the recognized reasons to stay out, since heat raises heart rate and drops peripheral resistance at once. A long-standing review in the American Journal of Medicine names both, and concludes that sauna bathing is otherwise well tolerated by most healthy adults. Stable coronary disease is a different case: 21 people with it sat in a sauna four times a week for two months in the trial above without the study reporting harm.

Stand up slowly on the way out, because your blood pressure really is lower than when you went in. If you are pregnant, on medication affecting blood pressure or fluid balance, or managing a heart condition, get individual advice rather than extrapolating from a cohort of Finnish men.
The Bottom Line
If you already have easy access to a sauna, four sessions a week at 15 to 20 minutes is a good habit. The downside is close to zero, the blood pressure effect is real if small, and the observational signal, however confounded, points the same way every time anyone looks.
What you should not do is buy a sauna on the strength of a 40 percent mortality reduction. That figure describes Finnish men who were already regulars, not what happens when you start, and the trials that assigned people to heat found a few millimeters of mercury and little else. It is the same gap between a striking association and a disappointing trial that runs through the omega-3 index and the studies that tested it. Go four times a week if you can manage it, go because you will keep going, and treat the longevity headline as grounds for optimism rather than a promise.
